Maybe it could start huge and people could be killed off when they stop putting as much time in/split off into different travelling groups due to irreconcilable differences
You could make the game really hard and have it so people perma-die if they fuck up. It would certainly add to the fun honestly. I don’t know if that’s a thing in DnD though.
